(Nasdaq:IMGN), a leader in the expanding field of antibody-drug conjugates (ADCs) for the treatment of cancer, today announced that results from the Phase 1 expansion cohort evaluating mirvetuximab soravtansine (IMGN853) in patients with folate receptor alpha (FRα)-positive platinum-resistant ovarian cancer were published in the Journal of Clinical Oncology. The data demonstrate the potential clinical benefit of mirvetuximab soravtansine for the treatment of platinum-resistant ovarian cancer.

      “Standard single-agent therapy for patients with platinum-resistant ovarian cancer typically has a response rate below 20% and median progression-free survival below four months,” said Kathleen Moore, M.D., Associate Professor, Stephenson Cancer Center, University of Oklahoma, and lead author of the publication. “Mirvetuximab soravtansine generated encouraging efficacy and tolerability data in the Phase 1 trial that suggest the potential to improve clinical outcomes for this patient population.”

      The Phase 1 expansion cohort enrolled 46 patients with platinum-resistant epithelial ovarian, fallopian tube, or primary peritoneal cancer whose tumors were positive for FRα. Patients were dosed with mirvetuximab soravtansine once every three weeks. Mirvetuximab soravtansine demonstrated single-agent activity in the 46-patient cohort with a 26% confirmed response rate and median progression free survival (PFS) of 4.8 months. In a subset of 23 patients with low, medium or high FRα, who had received three or fewer prior lines of therapy, there was a 39% objective response rate (ORR) and median PFS of 6.7 months. On the basis of the study findings and additional data demonstrating the importance of FRα expression levels with mirvetuximab soravtansine, the Company has designed the Phase 3 FORWARD I study to enroll patients with platinum-resistant ovarian cancer with one to three prior therapies and with medium or high FRα. This group of patients in the Phase 1 expansion cohort exhibited a 44% ORR and a median PFS of 6.7 months.1

      Mirvetuximab soravtansine exhibited a manageable safety profile. Adverse events (AEs) were generally mild with the majority being grade 1 or grade 2 (least severe grades). The most commonly observed AEs were diarrhea, blurred vision, nausea, and fatigue.

      “These results demonstrate that mirvetuximab soravtansine is active in platinum-resistant ovarian cancer, with encouraging response rates and progression-free survival combined with a manageable safety profile,” said Anna Berkenblit, M.D., Vice President and Chief Medical Officer of ImmunoGen. “On the basis of these findings, we have moved confidently into a Phase 3 registration study evaluating this promising agent against the standard of care in the platinum-resistant setting. In addition, we are evaluating combination regimens to assess mirvetuximab soravtansine in expanded patient populations and will begin reporting data from these combinations in mid-2017.”

Es kann durchaus schon vorher zu statistischen Häufungen gekommen sein, die man noch als Zufall abtun konnte. Aber (waren es?) 4 Todesfälle binnen kürzerer Zeit sind halt schwer durch Zufall zu begründen....

      Was sehr verwundert: Vor drei Wochen sind beim ASH-Meeting jede Menge Daten zu den laufenden SGN-CD33A-Studien (SGN-CD33A befindet sich seit 2013 in der klinischen Testung) präsentiert worden, von Leberproblemen war in der zusammenfassenden Unternehmensmeldung keine Rede -> http://investor.seattlegenetics.com/phoenix.zhtml?c=124860&p…
      Zufälle soll´s ja geben, alle Probleme inklusive der Todesfälle müssen erst kurz vor Weihnachten aufgetreten sein ...

      Die Frage ist zweifellos berechtigt, zumal schon bei Mylotarg (anti-CD33-ADC von Wyeth, wurde in den USA vom Markt genommen, in der EU erst gar nicht zugelassen) Leberprobleme aufgetreten waren (Lebertoxizität und sinusoidales Obstruktionssyndrom = Lebervenenverschlußkrankheit). Vermutlich kommen da mehrere Faktoren zusammen: Grunderkrankung AML + Abbau des ADC via Leber etc.


      Für mich stellt sich die Situation aktuell so dar:

      IMGN799 wurde bislang in Analystenstudien mit 0 USD bewertet, während SGN-CD33A aufgrund bisheriger Ergebnisdaten mit einem Milliardenbetrag bewertet worden ist. Und in der Tat habe auch ich mir die Frage gestellt, mit welchem Ziel IMGN zeitverzögert die Entwicklung eines anti-CD33-Produkts in Angriff genommen hat, wo doch die SGN-CD33A-Daten bislang überzeugend waren. Seitens IMGN ist dies mit einem laut Präklinik weitaus günstigerem therapeutischen Fenster für IMGN799 begründet worden. Das therapeutische Fenster des SGN-CD33A sah man als zu gering an.

      Zumindest mit letzterer Einschätzung lag man wohl richtig. Das bedeutet natürlich noch lange nicht, dass IMGN799 funktionieren wird. Dennoch dürfte nun der Einwand eines weit fortgeschrittenen, aussichtsreichen Konkurrenzpräparates wegfallen. Insoweit scheint die Entwicklung des IMGN799 nunmehr rational nachvollziehbar. Entscheidend wird sein, ob das therapeutische Fenster tatsächlich groß genug ist, um bei überzeugender Wirksamkeit die bei den Konkurrenzpräparaten beobachteten Nebenwirkungen zu vermeiden. Als Außenstehender läßt sich das derzeit nicht beurteilen.


      "Der Markt" schwankt in seiner Einschätzung offensichtlich hin und her. Zwischenzeitlich deutlicher Kursanstieg der IMGN-Aktie auf 1,78 USD (-> Markt sieht Chancen für IMGN779), danach Einbruch auf 1,59 USD (-> Markt überträgt SGN-CD33A-Probleme auf IMGN779), aktuell notiert die IMGN-Aktie wieder auf dem Ausgangsniveau. SGEN hingegen gibt um mehr als 15 % nach, was angesichts der aktuellen Meldung und dem Kursanstieg der vergangenen Monate kein Wunder ist.
