TOP-ART-Studie
Die Phase 2-Studie prüft bei Patienten mit molekulargenetisch nachgewiesenen Störungen in der DNA-Reparatur die Therapie mit dem PARP-Inhibitor Olaparib in Kombination mit dem Medikament Trabectedin.
Jeder Krebs geht auf Erbgutveränderungen zurück. Meist sind sie im Laufe des Lebens in einzelnen Körperzellen entstanden, seltener von den Eltern vererbt. Zu den Erbgutveränderungen kommt es durch verschiedene Mechanismen, etwa durch Kopierfehler bei der DNA-Verdoppelung oder durch defekte DNA-Reparaturenzyme. Metastasierte solide Tumoren weisen häufig Veränderungen in Genen auf, die für die Reparatur von genetischen Schäden wie zum Beispiel DNA-Doppelstrangbrüchen entscheidend sind. Solche für die Zelle essentielle Gene sind unter anderem BRCA1/2, CHEK2 und PALB2. Ihre eigentliche Aufgabe ist es, durch die sogenannte „homologe Rekombination (HR)“ Fehler im Erbgut der Zelle zu beheben. Dabei tauschen sie genetisches Material zwischen den sich entsprechenden, also homologen Chromosomen des doppelten Chromosomensatzes oder zwischen verdoppelten Schwester-Chromatiden in der Phase der Zellteilung aus.
Bei Ausfall der Reparaturenzyme der HR ist die Krebszelle in der Regel von einem weiteren DNA-Reparaturweg abhängig, der ihr das Überleben sichert. Dieser zweite Weg ist das Reparaturenzym PARP1. Sind beide Reparaturmechanismen, also HR und PARP1 blockiert, ist dies für die Tumorzelle in der Regel tödlich.
An diesem Punkt setzt die TOP-ART-Studie – eine sogenannte „Investigator-Initiated Trial“ (siehe Infobox) – an, um einen neuen Behandlungsansatz für Krebspatienten mit lokal fortgeschrittenen bzw. metastasierten soliden Organtumoren zu prüfen. Die Phase 2-Studie schließt Patienten ein, die bereits defekte Gene im HR-Reparaturweg wie z.B. BRCA1/2 aufweisen. Damit haben die Krebszellen nur noch eine Möglichkeit, um die Schäden in der Zelle über PARP1 zu korrigieren. Mit der Kombinationstherapie aus zwei Medikamenten möchte man die Reparaturwege der Krebszelle bei diesen Patienten nun komplett ausschalten und so gezielt die Tumorzellen abtöten. Das eine Medikament (Trabectedin) verursacht zusätzliche genetische Schäden in Form von DNA-Doppelstrangbrüchen, die andere Substanz (Olaparib) hemmt das Reparaturenzym PARP1.
Der PARP-Hemmer Olaparib (Lynparza®, AstraZeneca) wurde für die Behandlung von Patientinnen mit BRCA1/2-mutierten Ovarialkarzinomen zugelassen. Trabectedin (Yondelis®, PharmaMar) ist für die Therapie von Patienten mit Weichgewebesarkomen und platinsensitiven Ovarialkarzinomen zugelassen. Auf Grundlage der bisherigen Erfahrungen mit den Medikamenten im experimentellen Arm erwarten die Wissenschaftler und Ärzte ein Abtöten der Krebszellen. Die Kombinationstherapie mit Olaparib und Trabectedin erfolgt in wiederholten Zyklen von drei Wochen.

New Zealand and other Southeast Asian countries.
Professor Jeff Szer, Peter MacCallum Cancer Centre and Royal Melbourne Hospital
haematologist, who was the Australian principal investigator on the pivotal Aplidin®
registration study, said Aplidin® had been shown to be effective and well tolerated.
He commented: “More Australian myeloma patients were enrolled into the pivotal
international trial of Aplidin® than anywhere else in the world. These patients in the
Phase 3 study known as ADMYRE have now paved the way for others to have access
to a new and novel therapy. This really means that some patients with advanced
myeloma have the possibility of improved outcomes, when previous therapies have failed.”

production processes and designed to accelerate manufacturing and improve yields
in the face of new potential needs.
This new authorization is to be added to those already existing at PharmaMar’s
facilities, that also have the approval from the European Medicines Agency (EMA),
the US FDA and the Japanese agency (NIHS) for the production of the active
pharmaceutical ingredients (API) of their compounds.

The company develops RNAi therapies — administered as eye-drops – to combat eye diseases such as dry eye disease, glaucoma and retinal diseases, which cumulatively affect hundreds of millions of people across the globe.
“So far, the goal of dry eye disease treatments has been to relieve its symptoms. Artificial tears are used to help lubricate the eye and need to be applied several times a day,” explained Ana Isabel Jiménez, COO and R&D Director of Sylentis.
“That is why Sylentis is busy researching for ways to improve treatment and make it much more effective, using the mechanism of RNA interference which makes it possible to control and reduce the levels of abnormal proteins that would naturally improve eye hydration and thus reduce the pain and discomfort of dry eye.”
Sylentis is pushing ahead with its R&D in the wake of Onpattro’s approval: the company has a dry eye disease treatment currently in Phase III, a glaucoma treatment in Phase II, and a promising retina disease candidate that they hope will enter clinical development next year.

PharmaMar announces the meeting of its patient recruitment target in the phase II study with lurbinectedin as a single agent in small-cell lung cancer

with lurbinectedin as a single agent in small-cell lung cancer (SCLC) has been achieved
• The study’s primary endpoint is overall response rate.
Other secondary endpoints such as the duration of response, progression
free survival, overall survival, along with the safety profile will also be evaluated
This multicenter, phase II “basket”, clinical trial, involving 38 centers from nine
different countries, will assess the safety and efficacy of lurbinectedin in patients with
recurrent SCLC, this meaning, those patients who have received a prior
chemotherapy treatment.
In June, PharmaMar presented data on 61 patients from this cohort with recurrent
small-cell lung cancer at the American Society of Clinical Oncology (ASCO) Congress
in the abstract entitled "Efficacy and safety of lurbinectedin (PM1183, Zepsyre®) in
small-cell lung cancer (SCLC): results from a phase 2 study". An ORR of 39.3% was
seen in these patients. The median DR was 6.2 months and the median OS, 11.8 months.
"We have observed that lurbinectedin as a single agent is active in patients with
recurrent small-cell lung cancer and that, according to the data observed so far, there
