habe ich so das Gefühl (nicht mehr!), dass die auf eigene Faust (wie
seinerzeit Biogen) das Symlin vertreiben wollen. Ob das gut oder schlecht ist, vermag
ich nicht einzuschätzen, auf jeden Fall ist es die riskantere Variante.

Hallo ihr zukünftigen AMYLIONÄRE;
Die Aussichten von Amylin sind wirklich rosig, denn sollte das Präparat
Symlin das FDA Approval schaffen, sind wir ganz schnell bei 70/90, sollte
es Mitte 2000 noch nicht klappen, wird der Kurs zwar dahinsiechen, die
Chancen bleiben aber erhalten.
Der bisherige Chartverlauf läßt (fast) keine Wünsche offen, wenn man die
Aktie schon einige Zeit hat. Die jetzige Situation ist kritisch und wird
sich allein durch die bald verkündeten Ergebniszahlen entscheiden.
Ich bin mit einer kleinen Postition investiert und beobachte AMLN täglich.
Bei Ausbruch würde ich sofort zukaufen, und meinen Stop setze ich bei
9$.

Wenn ihr Euch ein Stop-Loss-Limit gesetzt habt, dann müßt ihr das auch einhalten, sonst verletzt man die eigenen Strategie.
Persönlich habe ich kein Stop-Loss, denn was einige Chaoten und Shorties(!) mit diemsem Wert machen ist mir ziemlich egal.
Mit den Fakts hat das jedenfalls alles nichts zu tun.
Vor einigen Tagen gab es eine Kaufempfehlung mit Kursziel 40 $ auf ein Jahr von Meg Malloy, DIE Biotech-Analystin von H&Q.
Eigentlich gibt es keine wichtigere Stimme im Biotech-Bereich, allerdings gehen solche Empfehlungen in dieser Börsenphase völlig unter, dass ist typisch.

Interessant sind folgende Thesen:
Symlin wird problemlos für Typ 1 Diabetes mellitus zugelassen werden, für Typ 2 wird es wegen der vorhandenen Konkurrenz schwierig wird.
Ich persönlich sah die FDA-Zulassung als kritischsten Punkt für Symlin an, insofern ist ihr Optimismus sehr ermutigend!
Andererseits stehen die Konkurrenzprodukte momentan enorm unter Beschuss, da Rezulin jüngst wegen Leberschäden als Nebenwirkung vomm US-Markt gezogen wurde.
Viele Konkurrenzprodukte sind chemisch verwandt mit Rezulin. Und Symlin erzeugt als einziger Insulinzusatz keine Unterzuckerung, das ist ein sehr wichtiger Punkt!
Weiter schätzt Meg Malloy das Marktpotential auf 700 Mio $, dass sind fast Biogen/Avonex-Maßstäbe!! Vergleicht dann mal die Marketcap von Biogen/Amylin.
Und die Pipeline von Amylin ist mit Exendin wesentlich interssanter!

Investment Thesis and valuation
After over a decade of development effort on Symlin, Amylin is now at a turning point. The company has completed 6 Phase III studies and over twenty
Phase II studies with Symlin in the Type 1 and Type 2 diabetes settings. While the overall effect of Symlin is modest and there is risk concerning regulatory approval, we believe that the odds favor success. We believe that the regulatory and commercial hurdles will be lower for Type 1 diabetes, which we believe represents a commercial opportunity in excess of $700M in the U.S. alone. The commercial opportunity in the Type 2 diabetes setting could be twice as large, however, we believe that the regulatory and commercial risks in this indication are higher. With the recent completion of a $100M private placement, Amylin is in a unique position for a biotech company. The pivotal studies for Symlin are completed and paid for. The company has the financial strength to establish an attractive corporate partnership for Symlin. If
Symlin is approved, we believe that a fair, one-year price target for Amylin would exceed $40 per share.

Clinical data Summary
Symlin (Pramlintide acetate) overview
Symlin is a synthetic form of a natural hormone called amylin, which is thought to act in concert with insulin to regulate glucose metabolism. Like insulin, amylin is
secreted by the pancreatic beta cells. Amylin is thought to modulate the delivery of nutrients to the small intestine and to suppress glucagon secretion and hepatic
glucose production.
Vast database and good safety package
The company has conducted a total of 6 pivotal studies; three in Type 1 patients and three in Type 2 patients (Exhibit 2). In addition to the Phase III studies, Amylin has conducted over 20 Phase II studies with Symlin. Over 4,200 patients have been treated with Symlin in clinical studies. In aggregate, the company has collected data on 2,500 patient years. The company has data on over 380 patients going out for two years.

Conclusions from clinical studies
Studies conducted to date have assessed different dosing levels and regimens, and have involved somewhat heterogeneous patient populations. While the studies and
results have been complex, the efficacy of Symlin has been quite consistent. We draw the following conclusions.
*The magnitude of Symlin`s overall effectiveness is modest. The primary measure of efficacy is HbA1c, a measure of long term glucose metabolism. On an intent-to-treat basis, treatment with Symlin results in HbA1c improvements ranging between 0.3% and 0.4% at 6 and 12 months in Type 1 and Type 2 patients (see exhibit 3). In addition, Symlin treatment has been associated with consistent improvements in weight control.
*Efficacy is more profound among certain subsets of responder patients, who can be identified after four weeks. We believe that the responder data will be important
for clinicians in judging whom to treat. However, we believe that the intent to treat analysis will serve as the basis for approval. While the results were not robust, they
were statistically significant.
*Based on a solid safety profile, unique mechanism and incremental benefit, we believe that Symlin may have clinical utility in a subset of both Type 1 and
Type 2 patients.
*Based largely on the results of the landmark Diabetes Control and
Complications Trial (DCCT) in Type 1 patients, the UKPDS in Type 2 patients, as well as FDA panel discussions on guidelines for reviewing new diabetic agents, we believe that clinicians have come to regard any improvement in HbA1c as clinically relevant. This fact, coupled with a broad safety package, may tip the odds toward approval
*We believe that the hurdles for approval and potential use will be lower for
Type 1 patients because other than improved insulin, there have been no new therapies for this patient population.
*The hurdles for Type 2 diabetes approval and usage are higher, because several oral agents, some of which are perceived to be more efficacious, are available.
However, many patients lose their responsiveness to available oral agents. Symlin acts by a unique mechanism and may have utility as concomitant therapy in some Type 2 patients. Moreover, most available agents are associated with significant side effects, and Symlin`s safety profile will likely be perceived as an advantage toward approval.

Symlin (Pramlintide acetate) overview
Symlin is a synthetic form of a natural hormone called amylin, which is thought to act in concert with insulin to regulate glucose metabolism. Like insulin, amylin is
secreted by the pancreatic beta cells. Amylin is thought to modulate the delivery of nutrients to the small intestine and to suppress glucagon secretion and hepatic
glucose production.
Vast database and good safety package
The company has conducted a total of 6 pivotal studies; three in Type 1 patients and three in Type 2 patients (Exhibit 2). In addition to the Phase III studies, Amylin has conducted over 20 Phase II studies with Symlin. Over 4,200 patients have been treated with Symlin in clinical studies. In aggregate, the company has collected data on 2,500 patient years. The company has data on over 380 patients going out for two years.

Conclusions from clinical studies
Studies conducted to date have assessed different dosing levels and regimens, and have involved somewhat heterogeneous patient populations. While the studies and
results have been complex, the efficacy of Symlin has been quite consistent. We draw the following conclusions.
*The magnitude of Symlin`s overall effectiveness is modest. The primary measure of efficacy is HbA1c, a measure of long term glucose metabolism. On an intent-to-treat basis, treatment with Symlin results in HbA1c improvements ranging between 0.3% and 0.4% at 6 and 12 months in Type 1 and Type 2 patients (see exhibit 3). In addition, Symlin treatment has been associated with consistent improvements in weight control.
*Efficacy is more profound among certain subsets of responder patients, who can be identified after four weeks. We believe that the responder data will be important
for clinicians in judging whom to treat. However, we believe that the intent to treat analysis will serve as the basis for approval. While the results were not robust, they
were statistically significant.
*Based on a solid safety profile, unique mechanism and incremental benefit, we believe that Symlin may have clinical utility in a subset of both Type 1 and
Type 2 patients.
*Based largely on the results of the landmark Diabetes Control and
Complications Trial (DCCT) in Type 1 patients, the UKPDS in Type 2 patients, as well as FDA panel discussions on guidelines for reviewing new diabetic agents, we believe that clinicians have come to regard any improvement in HbA1c as clinically relevant. This fact, coupled with a broad safety package, may tip the odds toward approval
*We believe that the hurdles for approval and potential use will be lower for
Type 1 patients because other than improved insulin, there have been no new therapies for this patient population.
*The hurdles for Type 2 diabetes approval and usage are higher, because several oral agents, some of which are perceived to be more efficacious, are available.
However, many patients lose their responsiveness to available oral agents. Symlin acts by a unique mechanism and may have utility as concomitant therapy in some Type 2 patients. Moreover, most available agents are associated with significant side effects, and Symlin`s safety profile will likely be perceived as an advantage toward approval.

Allerdings ist bei Biotech-Investing eigentlich Englisch Pflicht, da die wichtigen Nachrichten immer in Englisch abgefasst werden! Meg Malloy stuft Amylin auf Kauf herauf wegen der guten Aussichten für die Zulassung von Symlin, der deutlich verbesserten Geldbestände von Amylin gegenüber den letzten Jahren und deren sich nun schneller entwickelnden Produktpipeline.
Amylin wird wohl im Sommer 2000 die Zulassung von Symlin für den amerikanischen Markt und ein wenig später auch für den europäischen Markt beantragen. Die Entscheidung der zuständigen Behörden (in USA die FDA) darüber wird wohl 2001 gefällt werden

Sie glaubt, dass die Zulassungshürde für Diabetes 1 mangels Konkurrenzprodukt besonders gering sein dürfte und rechnet dann mit mit Umsätzen allein in USA für Diabetes 1 von bis zu 700 $ Millionen/Jahr (das sind enorm große Zahlen!). Sollte Diabetes Typ 2 auch zugelassen werden, dann wären die möglichen Verkaufszahlen doppelt so groß. Allerdings sind die Zulassungshürden und die kommerziellen Risiken hier höher. Zusammen mit der guten Finanzbasis hätten sie damit eine gute Verhandlungsbasis für Gespräche mit einem großen Pharmariesen wie Novo Nordisk Eli Lilly oder Novartis.
Sofern Symlin zugelassen wird , ist ihr Preisziel in einem Jahr 40 $ .
Die 100 Millionen Dollar lassen auch eine schnellere Entwicklung der Pipeline zu.
Zum einen das Exendin(=AC2993) für nicht insulinpflichtige Typ Diabetiker, welches sich in Phase II befindet. Amylin läßt sich bei dem Zulassungsprozeß noch mehr Zeit, dh. erst im letzten Quartal dieses Jahres werden die die Zulassung von Symlin beantragen. Sinn der Verzögerung ist es, ein Dareichungsprotokoll mit anderen Produzenten von Pharmaka/Dareichungsappllikationen abzustimmen.
Je nachdem , ob Symlin von der FDA einen "fast track status" erhält, dauert der gesamte Zulassungprozeß, wenn nichts dazwischenkommt, bis Mitte 2001 bzw Ende 2001..
Ich fürchte, da werden sich in der Zwischenzeit viele Aktionäre langweilen, und der Kurs wird vor sich hindümpeln bzw leicht fallen.
Bisher wurden ja leider auch die positiven Phase 2-Ergebnisse noch nicht honoriert, so dass Exendin wohl auch nicht für bessere Stimmung sorgen wird.

Amylin Pharmaceuticals Announces Initiation of Phase 1 Clinical Study For Atherosclerosis/Restenosis Drug Candidate
SAN DIEGO, June 1 /PRNewswire/ -- Amylin Pharmaceuticals, Inc. (Nasdaq: AMLN - news) today announced the initiation of the first Phase 1 clinical trial for its drug candidate AC3056. The development program for AC3056 will target the prevention of restenosis following angioplasty and other procedures to open clogged arteries, and the treatment of atherosclerosis. The safety and tolerability study is being initiated following the required regulatory waiting period after submission of an Investigational New Drug (IND) application to the Food and Drug Administration (FDA) in late April.
Atherosclerosis, the underlying cause of coronary artery disease and stroke, is characterized by cholesterol-rich plaque in the wall of arteries. Atherosclerotic plaque formation involves multiple processes, and the risk for atherosclerosis is increased by high levels of low density lipoprotein (LDL) or ``bad`` cholesterol. Restenosis is the re-narrowing of blood vessels following procedures employed to increase blood flow in diseased arteries.
AC3056 is a compound that has demonstrated vascular protective properties in animals. The compound is being evaluated as a potential treatment to prevent restenosis after vascular repair procedures and for the treatment of diseases resulting from atherosclerosis. Preclinical data indicate multiple modes of action on a cascade of events generally thought to be involved in the initiation and growth of atherosclerotic lesions. These data indicate that AC3056 is an antioxidant that inhibits vascular cell adhesion molecule expression in human vascular cells. In animal models of atherosclerosis, AC3056 was orally active, lowered serum cholesterol concentrations, inhibited the formation of atherosclerotic plaques in the arterial wall and prevented cholesterol-induced damage to vascular function.
Coronary artery disease is the single leading cause of death in industrialized countries, responsible for approximately 500,000 deaths in the United States annually. Restenosis is the major limitation to long-term clinical success of angioplasty and other procedures employed to open clogged arteries. The American Heart Association estimates that restenosis rates associated with coronary angioplasty range from 25 to 30 percent within 6 months. Coronary bypass grafts are also susceptible to high rates of restenosis. Over three-quarters of a million cardiology intervention procedures are estimated to have been performed in the United States in 1999.
``Despite the success of various revascularization procedures, restenosis remains a major limitation and a vexing problem,`` commented Alain D. Baron, MD, Vice President of Clinical Research at Amylin Pharmaceuticals. ``AC3056 is an exciting compound, as it displays an array of properties that may reduce restenosis and thus have a positive impact on the outcomes of angioplasty and other revascularization procedures.``

The development program for AC3056 will target the prevention of restenosis following angioplasty and other procedures to open clogged arteries, and the treatment of atherosclerosis. The safety and tolerability study is being initiated following the required regulatory waiting period after submission of an Investigational New Drug (IND) application to the Food and Drug Administration (FDA) in late April.
Atherosclerosis, the underlying cause of coronary artery disease and stroke, is characterized by cholesterol-rich plaque in the wall of arteries. Atherosclerotic plaque formation involves multiple processes, and the risk for atherosclerosis is increased by high levels of low density lipoprotein (LDL) or ``bad`` cholesterol. Restenosis is the re-narrowing of blood vessels following procedures employed to increase blood flow in diseased arteries.
AC3056 is a compound that has demonstrated vascular protective properties in animals. The compound is being evaluated as a potential treatment to prevent restenosis after vascular repair procedures and for the treatment of diseases resulting from atherosclerosis. Preclinical data indicate multiple modes of action on a cascade of events generally thought to be involved in the initiation and growth of atherosclerotic lesions. These data indicate that AC3056 is an antioxidant that inhibits vascular cell adhesion molecule expression in human vascular cells. In animal models of atherosclerosis, AC3056 was orally active, lowered serum cholesterol concentrations, inhibited the formation of atherosclerotic plaques in the arterial wall and prevented cholesterol-induced damage to vascular function.
Coronary artery disease is the single leading cause of death in industrialized countries, responsible for approximately 500,000 deaths in the United States annually. Restenosis is the major limitation to long-term clinical success of angioplasty and other procedures employed to open clogged arteries. The American Heart Association estimates that restenosis rates associated with coronary angioplasty range from 25 to 30 percent within 6 months. Coronary bypass grafts are also susceptible to high rates of restenosis. Over three-quarters of a million cardiology intervention procedures are estimated to have been performed in the United States in 1999.

irritieren würde, da es eine Widerstandslinie ist.

Lohnbüro, Verkaufs- und Presseabteilung abzuziehen, die ja nicht im eigentlichen Sinn produktiv sind.

Aber genau das könnten viele gemacht haben und sehen jetzt den Kurs abflitzen. Geholfen hat sicher auch die hohe Shortrate (3.25), die bei einem Kurs von 10 Euro ziemlich blödsinnig war.
"Vielleicht" haben Großinvestoren ja auch die jüngsten Phase2 -Ergebnisse analysiert und sind dann zu dem Schluß gekommen, dass Exendin vergleichbar oder sogar besser als die Konkurrenz wie Avandia ist. Dazu bin ich aber nicht beim besten Willen in der Lage.
Der Markt ist jedenfalls größer. Sehr, sehr groß
Was die Forschung von Amylin angeht, so kann man mit 30 Mann natürlich keine Grundlagenforschung machen. Amylins Politik war bisher, vielversprechende Ansätze von Universitäten zu lizensieren, und dann durch die klinischen Testreihen zu jagen.
Im Falle des Symlins und des Exendins nehme ich an, ohne es allerdings zu wissen, dass diese beiden Hormonderivate vmtl. in einer Forschungseinrichtung entwickelt wurden, und dann von einigen der beteiligten Forscher lizensiert wurden.
Diese gründeten dann mit Hilfe von Venture Kapital dann Amylin.
Seit Überschreiten der 500 Mio Market cap -Grenze werden die auch gecovered.

Wieso Tierversuch? Nicht Phase II Clinical Study?
In Phase 2 ist wird eine andere Anwendung untersucht: Der "simplen" Mehrausschüttung von Insulin durch die Langerhansschen Inseln bei Diabetes mellitus Typ 2, wenn der Glucoselevel zu hoch ist!
Bei den Tierversuchen geht es darum, aus anderen Bauschspeicheldrüsen-Zellen insulinproduzierende Langerhansssche Zellen mit Hilfe des Hormons Exendin zu erzeugen. Und wie gesagt, das hat wohl die Studie bei Tieren ergeben!
Das bedeutet, je nach Ausmass der Reaktivierung, Heilung von Diabetes mellitus Typ 2, nicht nur Behandlung! Typ 1 (Jugenddiabetes) ist wohl mehr eine Autoimmunerkrankung, aber wer weiss, vielleicht mit einer Kombitherapie mit immunsuppressiven Medikamenten...

Bedenke, dass die Substanzwerte der Biotechs wie Biogen und Biochem erst gerade angefangen haben zu rutschen. Es besteht noch echtes Abwärtspotential, obwohl es langfristig meinesachtens nicht gerechtfertigt ist. Wenn eine Biogen kracht, dann bleiben die kleinen leider auch nicht stehen.
Bei der Entscheidung der FDA über Amylin Ende November möchte ich auch nicht die Aktie halten.

Der HBA1c misst den durchschnittlichen Blutzuckerspiegel der letzten 50-80 Tagen, da die durchschnittliche Überlebensrate der roten Blutkörperchen bei 90-120 Tagen liegt. D.h. die volle Wirkung von Exendin ist noch nicht zum Tragen gekommen! All three AC2993 dosing regimens resulted in statistically significant reductions in glycated hemoglobin (HbA1c) and fructosamine, relative to placebo.
In this 28-day, triple-blind, placebo-controlled study, 109 participants with type 2 diabetes who were inadequately controlled with sulfonylureas and/or metformin (HbA1c greater than or equal to 8) were enrolled in 24 research centers in the US. Active arms received 0.08 micrograms per kilogram of AC2993 by subcutaneous injection two or three times a day. To maintain the study blind, patients treated twice a day received a third injection each day consisting of placebo. Participants maintained their current diabetes treatment regimens for the duration of the trial. Study assessments included changes from baseline in HbA1c, serum fructosamine (a clinically accepted measure of average glucose concentrations over approximately three weeks) and body weight.
AC2993 treatment led to statistically significant reductions in mean HbA1c (range of means: -1.1 to -0.7%) compared with placebo (-0.3%). Reductions in HbA1c of greater than or equal to 0.5% were achieved by 90% of AC2993 subjects compared with 33% of placebo subjects. The change in body weight between AC2993 (range of means: -0.5 to +0.2 kilograms) and placebo treated subjects (+0.9 kilograms) did not achieve statistical significance. The most common adverse event reported was mild to moderate nausea, consistent with previous AC2993 clinical studies.

Der blutzuckersenkende Effekt scheint wirklich enorm sein.

Aufgrund des deutlich niedrigeren HbA1c-Ausgangslevels (bei 8,2 %) der Avandia/metf.- Studie ist das aber Unsinn.

Zur zweiten Frage: Ich bin übrigens exakt in derselben Lage wie Du! Ich werde in den nächsten 30 Tagen auf jeden Fall einmal zwischenverkaufen, um die Gewinne erst einmal steuerfrei einzusacken. Steuerlich ist das einfach mal gegeben, denn volaltitle Zeiten kommen auf uns zu. Eine kleine Hype wäre mir deshalb jetzt ganz recht.
Mit einem nicht zu kleinen Teilbetrag werde ich dann Amylin zurückkaufen, denn die Konkurrenz von Exendin hat allesamt Verkaufszahlen oberhalb der Mrd $ Grenze. Und Exendin sieht im Vergleich dazu bisher alles andere als schlecht aus!
Wenn die Langzeitdaten (=1 Jahr) das auch bestätigen können, dann haben wir auch einen Blockbuster vor uns IMHO. Von der Geschichte, die ich am 19.3.2001 erwähnte, möchte ich gar nicht anfangen.

Jetzt gehts abwärts, die 13$ fällt gleich.
Was jetzt gerade abgeht sind kurzfristige Spielereien von kapitalstarken Zockern IMHO. Mit nix kann man Anleger mehr erschrecken als mit negativen Kursabschlägen folgend auf gute Nachrichten, weil es die Erwartungshaltung zerstört. So wie ich das so sehe, sind die Nachrichten gut (abgesehen eine gewisse Unsicherheit bzgl. der festgestellten Blutunterzuckerung = Hypoglykämie), aber ich bin auch kein Endokrinologe und bin selbst noch bei der Meinungsfindung.

Fonds kaufen nach und nach weiter, aber teilweise rührt sich der Kurs eben ein halbes Jahr auch gar nicht von der Stelle, dann fällt mal der Kurs um 10 % wie heute uswusf. Nie jedoch steigt der Kurs an einem Tag um 100 % aufgrund einer Nachricht, obwohl es sicher nur wenige (maximal 10) "critical news" gab.
Was ich damit aussagen will ist, dass ich es für möglich halte, dass Amylin nun folgend eine solche Form der Aufwärtsbewegung einnehmen kann. Ob da nicht irgendwo in den Daten nicht doch ein Wehrmutstropfen drin steckt, weiss ich nicht 100%. Da die Kurse aber nicht sofort nach Börsenbeginn fielen, sondern nur reaktiv fallen (man schaut, was die anderen machen), glaube ich nicht daran.

Ich muss feststellen, das ich die Wucht dieser Abwärtsbewegung völlig unterschätzt hatte. Speziell die Umsätze sind gewaltig, fast ein Drittel des Freeflows die letzten Tage. Das sind keine kleinen Jungs, die den Kurs anch unten bewegen. Alles Sicherungverkäufe der Fondmanager angesichts der FDA-Entscheidung? riesig gewordenene short-Verkäufe, die mit ihren Aktionionen sich den Kurs selbst vermasseln? Aber warum wird AMLN dann ihre Kapitalerhöhung noch zu 10 $ los?

Shares of Amylin Pharmaceuticals Inc. (AMLN) slipped more than 5% on heavy volume after Goldman Sachs issued a bullish long-term outlook on the stock, but said the company`s diabetes treatment, Symlin, faces a "short-term" risk.
The red flag appeared in a research note from Goldman biotechology analyst Meg Malloy, who initiated investment coverage of the stock with a market outperform rating. Amylin`s Symlin compound will go before a panel of the Food and Drug Administration for review July 26.
"While there is short-term risk on the potential approval of Symlin, scheduled for panel review on July 26, we believe that Amylin is well positioned to be a long term presence in the diabetes field," Malloy said.
Amylin has developed two late stage compounds for treating diabetes, including Symlin.
Amylin`s investor relations representative, Eric Shearin, said he think`s the company`s stock is in for a volatile month. "We are getting a lot of attention right now," he said. Shearing also said he thinks there "are a lot of short positions out there now." Short interest in Amylin`s stock rose nearly 48% in the month between May 15 and June 15, to 2 million shares from 1.4 million, according to the most recent data available from Nasdaq. Short interest represents the number of shares that have been reported as short in a stock. Short sellers borrow shares and sell them into the market on a bet that the price will decline. They then buy them back at a profit. Shares of Amylin were down recently 58 cents to $10.85 on volume of 2 million, compared with average daily volume of 644,873.
Fariba F. Ghodsian, of Roth Capital Partners, gives Amylin a neutral rating and said next month`s FDA meeting could go either way. "Our assessment for the meeting is 50-50 for a recommendation," she said. One drawback concerning the approval of Symlin is the complex statistical designs for the drug`s clinical trial {Stimmt}. agreement to collaborate on the development and commercialization of
Amylin`s compound AC2993 (synthetic exendin-4), a potential new
treatment for type 2 diabetes that, if approved, could represent the
first of a new class of compounds which have similar actions to GLP-1
(glucagon-like peptide-1). AC2993 is currently in Phase III clinical
trials with a submission to the U.S. Food and Drug Administration
anticipated as early as 2004.

Ich glaube da habe ich wohl was verpasst. Habe gerade mal meinen Aktienordner durchgestöbert und dabei festgestellt, dass ich am 21. April 99 1000 Stück Amylin Aktien mit viel Mühe noch zu 0,75 € verkauft habe. Seit dem habe ich den Wert nicht mehr beachtet. Kann mir jemand sagen, warum wir jetzt bei über 16 € angekommen sind

so ist das Leben. Ich hätte keinen Pfifferling auf die gegeben, die hatten aber das Glück die Krise mitten in der Aktie-Boomphase gehabt zu haben, so dass sie nochmal eine Studie starten konnten.
Das wird sicher die Meldung des heutigen Tages aus Biotechsicht. Was mir persönlich nicht gefällt ist die relativ hohe Zahl der Studienaussteiger (ZB wegen Übelkeit), dass kann bei der Zulassung ernsthafte Schwierigkeiten mit sich bringen. Wirken tut das Echsengift auf jeden Fall.
Mal sehen wie der Markt reagiert, ich bin mir da ehrlicherweise nicht ganz sicher.
Exenatide Open-Label Study Shows Encouraging Results in People With Type 2 Diabetes

Over 50% of Patients Reach Target A1C Levels, Despite Previous Failure With Oral Therapies
SAN DIEGO, and INDIANAPOLIS, June 15 /PRNewswire-FirstCall/ -- Amylin Pharmaceuticals, Inc., (Nasdaq: AMLN - News) and Eli Lilly and Company (NYSE: LLY - News) announced today that results from a Phase 3 open-label study of exenatide, a novel drug candidate for the treatment of type 2 diabetes, showed that more than half of the participants who completed 20 weeks of treatment achieved average glucose levels within the American Diabetes Association`s (ADA) target range. The findings from the ongoing study were presented today at the ADA`s 63rd Scientific Sessions in New Orleans, Louisiana.
Exenatide was added to the treatment regimens of 155 patients who had failed to reach target glucose levels on metformin, sulfonylurea, or a combination of the two. Prior to adding exenatide, the average level of hemoglobin A1c (A1C), a measure that provides a three-month average of glucose levels, was 8.6 percent. In the 63 patients who completed 20 weeks of treatment, the average A1C dropped 1.4 points to 7.2 percent, very near the ADA recommended target. In addition, exenatide decreased both fasting and post-prandial glucose levels.
"The fact that exenatide enabled so many patients not only to reduce their A1C but to reach the recommended target range is very encouraging," said Ralph DeFronzo, MD, MS, Professor of Medicine and Chief, Diabetes Division, University of Texas Health Science Center at San Antonio. "Reaching that target range is crucial to managing the disease and potentially preventing its devastating and deadly complications."
Exenatide (synthetic exendin-4) is being studied for its potential to address important unmet medical needs of many people with type 2 diabetes. Clinical trials suggest that exenatide decreases blood glucose toward normal levels. This control of blood glucose is likely due to actions of exenatide that are similar to those of the naturally occurring incretin hormone GLP-1. These actions are stimulation of the body`s ability to produce insulin in response to elevated levels of blood glucose, inhibition of the release of glucagon following meals, and slowing of gastric emptying. In animal studies GLP-1 administration resulted in preservation and formation of new beta cells, the insulin-producing cells in the pancreas that fail as type 2 diabetes develops(1,2,3,4).
"These open-label findings suggest exenatide could provide an important new treatment tool for physicians and patients struggling to reach target glucose levels but failing with current oral treatments," said Kenneth Polonsky, M.D., Adolphus Busch Professor and Chairman, Department of Medicine, Washington University in St. Louis. "If we continue to see these types of results in this and other studies, exenatide could offer new hope to millions of patients."
Key Study Findings
The findings from this open-label study are based on results from 63 patients who have been using exenatide for 20 weeks. These patients had been taking metformin, sulfonylureas or the combination of both metformin and sulfonylureas for at least three months prior to entering the study and had A1Cs between 7.5 and 12.0 percent. Researchers added twice-daily injections of exenatide to the patients` pre-study medication regimen, with an initial dose of 5 micrograms administered at breakfast and dinner, which was increased to 10 micrograms after four weeks. The most frequently reported adverse event was mild to moderate nausea, which decreased with continued treatment. Consistent with previous trials, mild to moderate hypoglycemia was observed in some patients taking sulfonylureas. Some emesis was also observed. Of the 77 subjects included in the 20-week analysis, the overall dropout rate was approximately 18 percent (n=14). The dropout rate due to nausea was less than 8 percent (n=6). Antibody formation was observed, however, the data indicate that there is no correlation between antibody formation and the effect on A1C. Additional data presented on these 63 subjects showed that the 1.4% reduction in A1C was sustained through 24 weeks of treatment with exenatide.

Exenatide Long-Acting-Release (LAR)
Specific data from a poster presentation at the ADA`s Scientific Sessions indicate that exenatide, when given in an extended-release formulation, continues to demonstrate a potent glucose-lowering effect as measured by A1C and fasting glucose levels in rats. An Investigational New Drug Application was submitted to the Food and Drug Administration (FDA) in March 2003 to support an independent program for exenatide LAR. The goal of the exenatide LAR program is to develop an extended-release, subcutaneous injection of exenatide, which could lead to weekly or monthly, rather than daily injections.
Exenatide Impact on Beta Cell Mass
An animal study presented at the conference further examined the impact of exenatide on beta cell function and mass. These study results confirmed that exenatide preserves beta cell mass and stimulates beta cell neogenesis through a mechanism of action that is independent of the potential benefits gained from the compound`s effect on weight loss and glucose control.
Diabetes -- The Need for New Treatments
Diabetes affects an estimated 177 million adults worldwide and more than 17 million in the United States(5). Approximately 90-95 percent of those affected have type 2 diabetes, in which either the body does not produce enough insulin or the cells in the body do not respond normally to the insulin. According to the US Center for Disease Control`s National Health and Examination Survey, 57 percent of diabetes patients do not achieve target A1C levels with their current treatment regimen and approximately 41 percent have A1Cs above 8 percent. According to the ADA, patients with A1Cs above target are more likely to develop diabetes-related complications, such as kidney disease, blindness and heart disease.

SAN DIEGO, and INDIANAPOLIS, June 15 /PRNewswire-FirstCall/ -- Amylin Pharmaceuticals, Inc., (Nasdaq: AMLN - News) and Eli Lilly and Company (NYSE: LLY - News) announced today that results from a Phase 3 open-label study of exenatide, a novel drug candidate for the treatment of type 2 diabetes, showed that more than half of the participants who completed 20 weeks of treatment achieved average glucose levels within the American Diabetes Association`s (ADA) target range. The findings from the ongoing study were presented today at the ADA`s 63rd Scientific Sessions in New Orleans, Louisiana.
Exenatide was added to the treatment regimens of 155 patients who had failed to reach target glucose levels on metformin, sulfonylurea, or a combination of the two. Prior to adding exenatide, the average level of hemoglobin A1c (A1C), a measure that provides a three-month average of glucose levels, was 8.6 percent. In the 63 patients who completed 20 weeks of treatment, the average A1C dropped 1.4 points to 7.2 percent, very near the ADA recommended target. In addition, exenatide decreased both fasting and post-prandial glucose levels.
"The fact that exenatide enabled so many patients not only to reduce their A1C but to reach the recommended target range is very encouraging," said Ralph DeFronzo, MD, MS, Professor of Medicine and Chief, Diabetes Division, University of Texas Health Science Center at San Antonio. "Reaching that target range is crucial to managing the disease and potentially preventing its devastating and deadly complications."
Exenatide (synthetic exendin-4) is being studied for its potential to address important unmet medical needs of many people with type 2 diabetes. Clinical trials suggest that exenatide decreases blood glucose toward normal levels. This control of blood glucose is likely due to actions of exenatide that are similar to those of the naturally occurring incretin hormone GLP-1. These actions are stimulation of the body`s ability to produce insulin in response to elevated levels of blood glucose, inhibition of the release of glucagon following meals, and slowing of gastric emptying. In animal studies GLP-1 administration resulted in preservation and formation of new beta cells, the insulin-producing cells in the pancreas that fail as type 2 diabetes develops(1,2,3,4).
"These open-label findings suggest exenatide could provide an important new treatment tool for physicians and patients struggling to reach target glucose levels but failing with current oral treatments," said Kenneth Polonsky, M.D., Adolphus Busch Professor and Chairman, Department of Medicine, Washington University in St. Louis. "If we continue to see these types of results in this and other studies, exenatide could offer new hope to millions of patients."
Key Study Findings
The findings from this open-label study are based on results from 63 patients who have been using exenatide for 20 weeks. These patients had been taking metformin, sulfonylureas or the combination of both metformin and sulfonylureas for at least three months prior to entering the study and had A1Cs between 7.5 and 12.0 percent. Researchers added twice-daily injections of exenatide to the patients` pre-study medication regimen, with an initial dose of 5 micrograms administered at breakfast and dinner, which was increased to 10 micrograms after four weeks. The most frequently reported adverse event was mild to moderate nausea, which decreased with continued treatment. Consistent with previous trials, mild to moderate hypoglycemia was observed in some patients taking sulfonylureas. Some emesis was also observed. Of the 77 subjects included in the 20-week analysis, the overall dropout rate was approximately 18 percent (n=14). The dropout rate due to nausea was less than 8 percent (n=6). Antibody formation was observed, however, the data indicate that there is no correlation between antibody formation and the effect on A1C. Additional data presented on these 63 subjects showed that the 1.4% reduction in A1C was sustained through 24 weeks of treatment with exenatide.

Exenatide Long-Acting-Release (LAR)
Specific data from a poster presentation at the ADA`s Scientific Sessions indicate that exenatide, when given in an extended-release formulation, continues to demonstrate a potent glucose-lowering effect as measured by A1C and fasting glucose levels in rats. An Investigational New Drug Application was submitted to the Food and Drug Administration (FDA) in March 2003 to support an independent program for exenatide LAR. The goal of the exenatide LAR program is to develop an extended-release, subcutaneous injection of exenatide, which could lead to weekly or monthly, rather than daily injections.
Exenatide Impact on Beta Cell Mass
An animal study presented at the conference further examined the impact of exenatide on beta cell function and mass. These study results confirmed that exenatide preserves beta cell mass and stimulates beta cell neogenesis through a mechanism of action that is independent of the potential benefits gained from the compound`s effect on weight loss and glucose control.
Diabetes -- The Need for New Treatments
Diabetes affects an estimated 177 million adults worldwide and more than 17 million in the United States(5). Approximately 90-95 percent of those affected have type 2 diabetes, in which either the body does not produce enough insulin or the cells in the body do not respond normally to the insulin. According to the US Center for Disease Control`s National Health and Examination Survey, 57 percent of diabetes patients do not achieve target A1C levels with their current treatment regimen and approximately 41 percent have A1Cs above 8 percent. According to the ADA, patients with A1Cs above target are more likely to develop diabetes-related complications, such as kidney disease, blindness and heart disease.

Ich komme da aktuell ins Grübeln, denn als ich hinschrieb dachte ich tatsächlich an Alterdiabetes. Hast mich wirklich in eine Verwirrung gestürzt.
Also fangen wir bei Altersdiabetes an:
Langjährig zu viel Zucker im Blut, um so mehr müssen die Betazellen Insulin herstellen. Die körpereigenen Körperzellen (spez. Muskeln) reagieren irgendwann mit einer Resistenz gegen Insulin und nehmen den Zucker nicht mehr auf. Blutzucker steigt weiter. Als Antwort wird immer mehr Insulin produziert, der Motor läuft auf Höchstlast im Leerlauf, bis die Betazellen "erschöpft" sind.
Die Sensitizer reduzieren die Resistenz, bei schlimmen Fällen muß aber schliesslich sogar Insulin gespritzt werden. Jetzt ist für mich die Frage, was ist "erschöpft". Sind die dann einfach kaputt oder bei Schonung (geringen Blutzucker zum Beispiel durch bewegung) vielleicht sogar reaktivierbar? Ich nix Ahnung

D.H. die körpereigene Abwehr greift die eigenen Betazellen an, weil sie als fremd erachten. In der Tat müssten Typ I Patienten bei einer Transplantation mit Stammzellen immer Immunsuppressiva nehmen, und auch das ist eigentlich kein schönes Leben. Genau deswegen habe ich nicht an Typ 1 gedacht.

Typ 1 ist wie Du richtig schreibst vermutlich eine autoimmunerkrankung, wobei man aber annimmt, dass die keorpereigenen betazellen durch eine an sich harmlose virusinfektion (etwa im kindesalter) an ihrer oberflaeche antigene exprimieren die dann die autoimmunreaktion triggern. Die vernichtung der betazellen ist dann ein je nach patient mehr oder weniger schleichender prozess der durchaus 20 jahre oder mehr brauchen kann. Der Diabetes manifestiert sich erst wenn die verbliebenen betazellen (etwa 10% oder so) nicht mehr genuegend Insulin produzieren(bei mir mit 34). Dann muss heute Insulin gepritzt werden. Koennte man aber betazellen nachzuechten und transplantieren kann es gut sein, dass diese wieder geraume zeit arbeiten bevor sie gekillt werden. Noch besser waere natuerlich wenn man sie so kapseln oder sonstwie abschirmen koennte, dass sie am leben bleiben. Bisher werden soweit ich weis nur ganze Spenderbauchspeicheldruesen tranplantiert und das macht man nur wenn eh eine Nierentransplantation faellig ist wegen der probleme mit der immunsuppression.
Beim "Alterszucker" (Typ2) ist wie Du auch ganz richtig schreibst die wirkung des insulins, naemlich membrankanaele zu oeffnen die glucose in die zellen schleusen reduziert (Resistenz). Dieser prozess der resistenz ist, soweit uns das seinerzeit der oberarzt in der schulung erzaehlt hat, recht komplex und noch nicht vollstaendig verstanden (gibt da auch eine erbliche Komponente). Man weis also nicht woher genau die resistenz kommt und auch nicht wie genau die sensitizer wirken. Ein problem ist aber sicher uebergewicht. Weiterhin ist bekannt, dass bei erhoehtem bedarf auch von alleine mehr membrankanaele bereit stehen. Das heisst fuer die oldies in erster linie sporten und abschlanken . Wenn sie das nicht schaffen oder das in verbindung mit den sensitizern nicht reicht kann man auch noch Insulin spritzen. Das problem sind aber hier nicht primaer die betazellen und die insulinproduktion. Kann sein, dass die Bauchspeicheldruese durch die ueberproduktion nachlaesst (erschoepfung) und dann fremdinsulin noetig ist. Es gibt aber auch sicher eine erholung der bauchspeicheldruese wenn der druck der ueberproduktion nachlaesst. Beim typ 1 heisst das honeymoonphase: da erholen sich die restlichen 10% betazellen nachdem insulin gespritzt wird soweit, dass ein paar monate spaeter der insulinbedarf erstmal deutlich sinkt und sogar auf null sinken kann. Weil beim typ 1 aber auch die verbleibenden betazellen weiter gekillt werden steigt dann der bedarf wieder an (wenn der honeymoon rum ist).
Zusammenfassend sind Deine ueberlegungen im wesentlichen richtig - bis auf den punkt mit der immunsuppression der wohl eher fuer verpflanzungen ganzer fremdorgane entscheidend ist. dennoch sind betazellen hauptsaechlich fuer Typ 1 interessant, weil sie da fehlen, waehrend fuer typ 2 interessanter ist warum das ja erstmal reichlich vorhandene insulin nicht wirkt wie es soll.

Mir war vaD nicht klar, dass die Pathogenese bei Typ 1 soo lange dauert. Ich denke bei Immunreaktion gerne an eine scharfe Reaktion. Hätte ich mir aber denken könne, bei MS ist ja auch nicht gleich das ganze Hirn lahmgelegt, die totale Rückbildung des Krankheitsbildes sogar üblich denn die Ausnahme. Schade wiederum, dass DBT1 nicht so verläuft.
Das prinzipielle Problem der Autoimmunkrankheiten wundert mich persönlich gar nicht, wenn man sieht, mit welcher Raffinesse Krebszellen sich vor dem Immunsystem tarnen. Ist das Immunsystem zu lasch gegen körperähnliche Zellen eingestellt, dann kommen wohl zu viele Tumorzellen über die kritische Masse. Und letztendlich gibt es zu fast jedem Organ eine Autoimmunerkrankung.
Mein Irrtum w.o kam daher, dass ich bei Transplantation von Zellen immer von Immunsuppressiva gelesen hatte (ich dachte auch bei den Stammzellen), aber auf die Stelle finde ich so einen Artikel nicht.
