... ahaaa
das einzige was mich beruhigt is
is natürlich en bisschen komplizierter
wie von mir dargestellt
aber
fakt is

Genta Receives Special Protocol Assessment from FDA for Randomized Trial of Genasense(R) Plus Chemotherapy in Chronic Lymphocytic Leukemia
Friday October 6, 8:00 am ET
BERKELEY HEIGHTS, N.J., Oct. 6 /PRNewswire-FirstCall/ -- Genta Incorporated (Nasdaq: GNTA - News) announced that the Company\'s proposal for a randomized trial of Genasense® (oblimersen sodium) Injection in patients with chronic lymphocytic leukemia (CLL) has completed Special Protocol Assessment (SPA) by the Food and Drug Administration (FDA). The SPA, which was submitted in December 2005, documents agreements regarding the trial\'s design and planned analysis. Genta has filed a New Drug Application (NDA) for Genasense plus chemotherapy, which is currently under review by FDA. The NDA has a target action date for a marketing approval decision by October 29, 2006 under the Prescription Drug User Fee Act (PDUFA).

The proposed trial, which will be conducted in symptomatic patients who have not previously received chemotherapy, would randomize patients to receive fludarabine plus rituximab (Rituxan®; Genentech, Inc.) with or without Genasense. The pivotal trial in the pending NDA employed a combination of fludarabine plus cyclophosphamide with or without Genasense in patients with relapsed or refractory CLL. The NDA was the subject of a review by FDA\'s Oncology Drug Advisory Committee (ODAC) on September 6, 2006 and did not receive a majority vote to approve the application.
"We are pleased that FDA had no outstanding issues regarding the design or analysis of this trial, and also with the Agency\'s agreement that there were no safety issues that precluded the use of Genasense in previously untreated patients," noted Dr. Raymond P. Warrell, Jr., Genta\'s Chief Executive Officer. "While the NDA remains under active review, we will provide Genasense at no cost to CLL patients in response to single-patient IND requests (also known as "compassionate use" requests). We will not make a decision whether to proceed with this new trial until we have had our requested meeting with FDA to review the status of the pending NDA."

@Richy

I was at the ODAC meeting. Genta did a good job unlike their melanoma meeting where there experts where prepared and argued in a positive manner.
In addition I work with clinicians and have discussed with them their assessment of the data.
The biggest problem is as follows:
The first question of that the FDA asked ODAC to consider was worded improperly from the basis of it was asking them not to evaluate the data in light of "accelerated approval" but was asking for an evaluation of the data in light of "full approval". I don't have the question here but when I go in the meeting I saw that as a huge issue. Dr. Itri, near the end made a reference to "accelerated approval" but the FDA boys slapped it down.
One of the doc's specifically questioned the meaning of the word "significantly" in the evaluation and others concurred that this was a large hurdle and having to meet that standard raises the hurdle higher (where accelerated approval requires a "belief" or "clinical benefit" be achieved.
The reality is, as evaluated from a full approval standpoint, the drug is not approvable and that is the argument the FDA made and seemingly wanted to stick to. The reality is though that there was a very convincing argument from an accelerated approval perspective that the
As it relates to safety there are definetly additional issues at it relates to Genasense use as it requires the infusion bags. That being the case the patients and docs did argue this point down.
Accelerated approval requires "promising products for life threatening diseases available on the market on the basis of preliminary evidence prior to formal demonstration of patient benefit"
This is clearly demonstrated from the data presented. The issue is will the FDA consider it in this light?
We shall see but the preliminary indications are no!

Genta veröffentlichte den "primary end point of the trial"
FDA.

Genta answers our questions about GenasenseIf you have been following this blog, you know I am on a campaign to get some answers from the FDA on how they evaluate potential new drugs. In this case, we are following the evaluation of a potential drug for my condition, CLL, and whether the evaluation is fair and based on the FDA and its outside consultants being conversant with the issues CLL patients and expert hematologists face every day. Basically, we are trying to a see if the process gives us patients appropriate opportunities to have potentially life-saving or life-extending drugs.
You may recall, I actually went to Washington, D.C. to testify as a patient advocate as an FDA advisory committee was evaluating Genta’s drug, Genasense. I came away feeling both the FDA presenters and several committee members were not giving the drug and the sponsor their due and that patients were losing in the process.
The FDA assured me they had consulted with CLL experts, even though a big lineup of them were there to support the application for approval and further study. The FDA would only tell me who they consulted with if the sponsor, Genta, okayed it. Genta did and yet the FDA still has been slow to give me any information. I heard from them today, and they said they had been delayed as they consulted with internal attorneys( Anwalt). They asked me to stay tuned.
However, on the Genta side, the company has provided me with detailed answers to sophisticated questions posed by readers of my blog, and I will excerpt them below. Many of you have written in with questions about Genta, Genasense, and their history with the FDA and study data. So, in an effort to keep you informed as we weigh in as patients, here goes:
Issue #1
Did Genasense fail to live up to expectations and provide only marginal benefit?
“20% difference was not the agreed upon goal for the Genasense CLL study. The ‘”goal”’ was to achieve a statistically significant difference in the CR/nPR endpoint. That goal was achieved. The 20% was an estimate used only for determining sample size – in this case we estimated that by examining historical information before we started the study that the control arm would yield a 24% response. Again, this estimate was based on historical CLL clinical trials data. The Genasense Phase 3 randomized clinical trial showed that the control arm response was actually only 7% in a rigorous study. This trial, being the first randomized study in relapsed or refractory CLL, used rigorous assessment criteria and therefore may have established a new benchmark for control arm response rate in CLL. In addition, the company estimated that Genasense would improve control results by a factor of 1.8. The trial showed Genasense improved that rate by a factor of 2.5, much larger than projected.”
Issue #2
Was the FDA and Genta Fighting About Confirmatory Study Details?
“There were no substantive outstanding issues on the SPA. The last item was an FDA request for more clarity on how the independent review for response would be conducted. There was of course, understanding that the trial would be confirmatory after accelerated approval, and that would still be the company’s intent.”
Issue #3
Was Genta and the FDA at odds about what the endpoints were supposed to be for the Study that was presented?
“The FDA agreed in writing that: “…durable complete responses supported by symptom benefit” are acceptable efficacy endpoints. (Dated from an FDA correspondence on 1/9/02). FDA stipulated that response rate, by itself, would not suffice for full approval. (Die FDA setzte die 20% in Eigenregie so hoch, das prüft der Anwalt) The company had always stipulated that we would be filing for accelerated approval, and that there were very substantial data to indicate CR/nPRs were highly beneficial for patients. We confirmed that finding in our trial.”
Whew! I know this is technical, but it speaks to the heart of issues that companies fight about with the FDA. It’s important for some of us with science backgrounds to try to understand and to monitor the process. Otherwise, there’s always the chance some potentially beneficial drug will be killed because a bureaucracy or a powerbroker lined up against it and we didn’t speak up.
I don’t mean to say the FDA is necessarily unfair. I just don’t know. I can’t wait to hear their side of this and I bet there are thousands of CLLers and other patients who are following this saga closely too.
In the end, we need to insist, as a community, to make sure we are protected. But we must also insist that we are given hope with new therapies and that our governmental processes are doing their very best for us, the taxpayers.

Does this Dr. Pazdur usually express such "bravado" at these meetings with statements like, No further study is needed, "what you see is what you get" shutting down further clarification and discussion?"
The FDA approved the SPA last Friday.
The FDA Commissioner has multiple complaints concerning Dr. Pazdur.
Dr. Pazdur is checking with his attorneys a month after ODAC to see if he can give out public information concerning consultants

Genasense in patients with advanced melanoma were published on-line this week in the Journal of Clinical Oncology, ahead of its print publication date of October 10, 2006.
The paper is accompanied by an editorial that discusses the trial's results in the context of current options for melanoma treatment. The extended follow-up data from this publication form the basis of a Marketing Authorization Application (MAA) that is currently pending review by the European Medicines Agency (EMEA).
"In our paper, long-term followup has confirmed the trends that were observed in the earlier analyses", said Dr. Agop Y. Bedikian,
dachte die meinten long therm mit 3 Jahres follow up! nicht die alte veröffentlichung mit 24 monaten

The extended follow-up data from this publication form the basis of a Marketing Authorization Application (MAA) that is currently pending review by the European Medicines Agency (EMEA).
"In our paper, long-term followup has confirmed the trends that were observed in the earlier analyses", said Dr. Agop Y. Bedikian,

Genta's manufacturing facilities passed an FDA inspection, and should Genasense receive approval, the company can make enough of the drug for a worldwide launch. The company has four products in very early stages of development. However, it's unlikely it will be able to come up with the necessary cash to fund those projects should Genasense fail to gain approval.
Even if Genasense hits the market soon, don't think that the money is simply going to flow freely into Genta's doors. Management has stated that the company is exploring nondilutive measures to help finance the launch of Genasense. That includes commercial borrowing and a partnership with a larger drug company.
Although Genta would have to give up some of the revenue, a partnership makes the most sense, as the company would be able to take advantage of a larger marketing and sales force and not take on debt. In its second-quarter conference call, Dr. Raymond Warrell Jr., Genta's CEO, said the company has enough cash to last into the first quarter. (Q1/2007)

Hi Spike,

http://www.genta.com/genta/Employment/careers.html
Dieser hier sieht besonders gut aus:
Product Manager (Job Code PM-2006)
Qualifications:
* Knowledge of product launch processes a plus.
* Oncology experience is a must including familiarity with the liquid tumor marketplace, especially Chronic Lymphocytic Leukemia (CLL).

Genasense(R) Receives Orphan Drug Designation in Australia for Patients With Advanced Melanoma
Tuesday October 17, 8:00 am ET
BERKELEY HEIGHTS, N.J., Oct. 17 /PRNewswire-FirstCall/ -- Genta Incorporated (Nasdaq: GNTA - News) announced that its lead anticancer drug, Genasense® (oblimersen), has received Orphan Drug designation from the Therapeutic Goods Administration (TGA), the regulatory authority in Australia. The designation was granted for the use of Genasense as treatment for patients with Stage IV malignant melanoma. Orphan Drug designation is intended to facilitate the development of drugs that address significant unmet medical needs, and it provides 7 years of market exclusivity after approval, grants and tax credits for research and development, and reduced filing fees for marketing applications.
"Melanoma is an especially serious public health problem in Australia," said Dr. Raymond P. Warrell, Jr., Genta's Chief Executive Officer. "The recently published Phase 3 study of Genasense plus dacarbazine included a large number of Australian participants and showed benefit across an array of clinical endpoints. Genta has retained a local agent in Australia to represent the Company before the TGA, and we are very pleased that the TGA has granted the requested designation in melanoma. We will decide whether to proceed with a formal marketing application pending further discussions with regulatory authorities."
Genta has filed a Marketing Authorization Application (MAA) that is currently under review with the European Medicines Agency (EMEA) for the use of Genasense plus dacarbazine for treatment of patients with advanced melanoma. That application, which follows the same format that would be required for an Australian submission, is comprised of results from a randomized multinational trial of dacarbazine with or with Genasense in patients with advanced melanoma. A summary of the published report of this pivotal trial can be accessed at: http://www.jco.org/cgi/content/abstract/JCO.2006.06.0483v1.

BOSTON (MarketWatch) -- Analysts at Rodman & Renshaw maintained their market perform rating on Genta Inc.(GNTA :
on Tuesday, citing the possibility that the Food and Drug Adminstration will issue an
approvable letter for its oncology drug Genasense, despite a negative recommendation on its drug application from an FDA advisory panel. The firm noted the company is trying to review its clinical data with the FDA before the application's decision deadline of Oct. 29. "We acknowledge that Genasense is an active drug and believe that the most likely outcome from the FDA meeting will be an approvable letter with the stipulation that another trial in patients who have relapsed from fludarabine therapy be evaluated before approval is granted," wrote analyst Ren Benjamin. "Even though the company recently raised funds of approximately $16 million, thereby extending the time before the company runs out of cash, we continue to advise investors to wait by the side-lines ahead of the final decision from both the FDA and EMEA (E.U. regulators) as further downside potential remains," he added.

@radic
