Iloperidone: Slam Dunk
(added 5/7/09)
Let's recap. A drug that has had four corporate sponsors; is barely distinguishable from a lesser-utilized existing drug other than by one of the lower profile side effects; was initially billed as being augmented by a genetic test which, it turns out, has no clinically relevant discriminatory power; appears to be less efficacious than the category leaders for the stated indication, for which many drugs are approved; generic competition has arrived and more is approaching; the NDA was responded to by the FDA with the request for an additional clinical study, which the Company then refused to do, instead resubmitting the NDA with more data from existing studies.
The result: The drug is given approval by the FDA. Slam dunk. Was there ever any doubt? Well, yes, and many observers, including NI, will be reviewing their analyses to see where we went wrong. There are a few hypotheses currently floating around regarding the reason for this approval:
A) The FDA loves the name Fanapt, they really love it. They'd approve mustard gas if it was called Fanapt.
B) Vanda has photos of various FDA officials utilizing a 'wide stance' at the Minneapolis airport.
C) Thomas Polymeropoulos Laughren and Robert Polymeropoulos Temple experienced a curious feeling of kinship and good will with Vanda's CEO.
D) Several key FDA regulators are troubled by akasthisia associated with their current antipsychotic regimen, and the prospect of a drug producing less restlessness was personally appealing.
E) Having been dealing with this drug since 1993, the FDA simply got sick of it. You're approved. Now go away.
It remains to be seen whether this unexpected regulatory success will be followed by commercial success. The schizophrenia area has Saphris (asenapine) and the resurrected Serdolect in the regulatory hopper, and the roster of major companies which will see Fanapt as something more than damaged goods is not long. Of course it only takes one, but particularly with the generic onslaught growing, there may not be much willingness to invest heavily in a launch for a product whose competitiveness is debatable at best.
This will give aid and comfort to those who believe that developing redundant drugs remains the most likely path to commercial payoff.

Titan Pharmaceuticals Inc.
SOUTH SAN FRANCISCO, Calif.--(BUSINESS WIRE)--Titan Pharmaceuticals, Inc. (Pink Sheets:TTNP - News) today announced the re-engagement of key prior senior executives of the company, effective May 17th, to manage the ongoing activities of Titan following the FDA’s marketing approval for Fanapt™ (Iloperidone), as well as to assist the board of directors in identifying and evaluating future plans. Marc Rubin, M.D. has been appointed as the Executive Chairman, Sunil Bhonsle has been appointed as the President and Kate Beebe, Ph.D. has been appointed as the Sr. Vice President of Clinical Development & Medical Affairs.
The board and the management recognize the limited financial resources available to the company at this time, and the employment agreements have been structured to minimize the use of cash until the receipt of royalty payments that are expected later this year or early next year. Dr. Rubin will receive stock options but will forgo a salary. Mr. Bhonsle will receive stock options and a reduced salary with all payments deferred until next year. Dr. Beebe will receive stock options and has also agreed to defer payment of a substantial portion of her salary until sufficient funds are available. Key elements of the executives’ employment agreements are summarized below. The directors of the company have also agreed to accept stock options in lieu of cash compensation owed to them.
“We are very pleased by the approval of Fanapt™, an important new treatment option for patients with schizophrenia. This also provides an opportunity to add substantial value to Titan,” said Marc Rubin, M.D., Executive Chairman of the company. “We will continue with our strategy to maximize the value of our current assets, including Probuphine®, for our shareholders while minimizing expenses,” he added.

I think the long run is going to prove that any purchase of shares of Titan Pharmaceuticals for under three dollars was a dang good one.

SOUTH SAN FRANCISCO, Calif., April 21 /PRNewswire-FirstCall/ -- Titan Pharmaceuticals, Inc. (Pink Sheets:TTNP.pk - News) today confirmed that the company expects to receive a payment of approximately $1.6 million by May 15, 2010 as the royalty on the first quarter 2010 sales of Fanapt™ (iloperidone), a drug for the treatment of schizophrenia launched in the US by Novartis Pharma AG in January 2010. First quarter sales of Fanapt were reported to be $21 million, and under the terms of its sublicense agreement with Novartis, Titan is entitled to receive royalties on net sales of Fanapt equal to 8% on annual net sales up to $200 million, and 10% on annual net sales above $200 million. Titan incurs no ongoing expenses associated with this potential future income.

Also sichere Einnahme für TTNP.

"Probuphine: Recent Events & Regulatory Path Forward
Probuphine is a novel formulation of buprenorphine that is capable of maintaining a stable, round the clock level of medicine in patients for up to six months following a single treatment. In July and August 2011, Titan announced positive top line results for its Phase 3 confirmatory study for the treatment of opioid dependence, as well as additional positive results demonstrating significant overall patient improvement, a compelling safety profile and non-inferiority to the approved drug Suboxone®.
In October 2011, Titan announced an update from its pre-NDA meeting with the FDA regarding the development of Probuphine for the treatment of opioid dependence. Titan believes that its Phase 3 clinical program completed to date is acceptable to the FDA to support submission of an NDA via the 505(b)(2) pathway and that no additional clinical efficacy or safety studies are required to support the submission. Titan received initial comments from the FDA regarding manufacturing and non-clinical information and is in ongoing discussions with the agency to confirm these requirements. The Company also received clear guidance from the FDA on the requirements for submitting an NDA for consideration to be designated as a Priority Review."

About Probuphine
Probuphine is a novel product that has completed Phase 3 clinical development for the long term treatment of opioid dependence and a New Drug Application (NDA) preparation is in process with submission expected in the third quarter of 2012. Probuphine is a subcutaneous implant, capable of delivering continuous and persistent, around the clock blood levels of buprenorphine for six months following a single treatment, enhancing patient compliance and retention. Buprenorphine, an approved agent for the treatment of opioid dependence, is currently available in the form of daily dosed sublingual tablet and film formulations with reported annual sales of over $1 billion in the United States. The safety and effectiveness of treatment with Probuphine has been demonstrated in several clinical studies conducted to date, including a 163-patient placebo-controlled study which demonstrated clinically meaningful and statistically significant treatment with Probuphine over a 24-week period and was published in the Journal of the American Medical Association (JAMA), and a confirmatory study of 287 patients that showed statistically significant efficacy versus placebo and non-inferiority with a currently marketed sublingual formulation of buprenorphine.
Probuphine was developed using ProNeura™, Titan’s continuous drug delivery system that consists of a small, solid rod made from a mixture of ethylene-vinyl acetate (EVA) and a drug substance. The resulting product is a solid matrix that is placed subcutaneously, normally in the upper arm in a simple office procedure, and is removed in a similar manner at the end of the treatment period. The drug substance is released slowly, at continuous levels, through the process of diffusion. This results in a constant rate of release similar to intravenous administration.

TITAN PHARMACEUTICALS RECEIVES COMPLETE RESPONSE LETTER FROM THE FDA FOR PROBUPHINE NEW DRUG APPLICATION
SOUTH SAN FRANCISCO, CA, April 30, 2013 – Titan Pharmaceuticals, Inc. (OTCBB: TTNP) announced that the U.S. Food and Drug Administration (FDA) has issued this evening a Complete Response Letter (CRL) to its New Drug Application (NDA) for Probuphine®, the company’s investigational subdermal implant for the maintenance treatment of opioid dependence in adult patients.
“Titan and our partner, Braeburn Pharmaceuticals, are extremely surprised and disappointed with the FDA’s response. Probuphine is a diversion-resistant formulation that is consistent with the recently-issued FDA guidance supporting diversion- and abuse-resistant products, and the NDA was designated Priority Review by the FDA. We believe Probuphine has demonstrated both safety and efficacy in accordance with primary endpoints that were pre-agreed with the FDA and, moreover, the safety, efficacy and overall approval of Probuphine was strongly supported by the Psychopharmacologic Drugs Advisory Committee,” said Marc Rubin, M.D., executive chairman of Titan Pharmaceuticals. “Given the nationally-recognized, growing and devastating opioid dependence epidemic, there is critical need for new safe and effective treatments that reduce the likelihood of abuse, diversion and accidental pediatric exposure, and Titan and Braeburn remain committed to making Probuphine available for patients that need it.”
The CRL states that the FDA cannot approve the application in its present form. The FDA has requested additional data supporting the efficacy of Probuphine, including:
•The ability of Probuphine to provide opioid blockade of relevant doses of agonists
•The effect of higher doses of Probuphine, ideally doses more closely approximating the blood plasma levels associated with sublingual doses of buprenorphine of 12 to 16 mg / day
•Human factors testing of the training associated with Probuphine’s insertion and removal
The CRL also included recommendations regarding product labeling and the implementation of the Risk Evaluation and Mitigation Strategy (REMS).
Titan and Braeburn Pharmaceuticals, which has licensed the commercialization rights for Probuphine in the U.S. and Canada, are committed to addressing the concerns raised by the FDA in the CRL. Titan will discuss with the FDA the scope of the CRL comments to obtain clarification and determine next steps.
About Opioid Dependence
According to recent estimates, there are 2.2 million people with opioid dependence in the U.S. Approximately 20 percent of this population is addicted to illicit opioids, such as heroin, and the other 80 percent to prescription opioids, such as oxycodone, hydrocodone, methadone, hydromorphone and codeine. Before the year 2000, medication-assisted therapies for opioid dependence had been sanctioned to a limited number of facilities in the U.S. The Drug Addiction Treatment Act of 2000 (DATA 2000) allowed medical office-based treatment of opioid dependence and greatly expanded patient access to medication-assisted treatments. As a result, an estimated 1.2 million people in the U.S. sought treatment for opioid dependence in 2011.
About Probuphine
Probuphine is an investigational subdermal implant designed to deliver continuous and persistent, around the clock blood levels of buprenorphine for six months following a single treatment, and to simplify patient compliance and retention. Buprenorphine, an approved agent for the treatment of opioid dependence, is currently available in the form of daily dosed sublingual tablets and film formulations, with reported 2011 sales of $1.3 billion in the United States.
Probuphine was developed using ProNeura™, Titan’s continuous drug delivery system that consists of a small, solid implant made from a mixture of ethylene-vinyl acetate (EVA) and a drug substance. The resulting construct is a solid matrix that is placed subdermally, normally in the upper arm in a simple office procedure, and removed in a similar manner at the end of the treatment period. The drug substance is released slowly and continuously through the process of dissolution resulting in a steady rate of release.
The efficacy and safety of Probuphine has been studied in several clinical trials, including a 163-patient, placebo-controlled study over a 24-week period (published in the Journal of the American Medical Association (JAMA)), and a confirmatory study of 287 patients designed to evaluate efficacy versus placebo, and non-inferiority with a currently marketed sublingual formulation of buprenorphine. Results of the confirmatory study were announced in July 2011.

Results of the confirmatory study were announced in July 2011.

http://www.nasdaq.com/press-release/titan-pharmaceuticals-re…
The FDA maintained the need for clinical data in this patient population as a requirement for approval, stipulating that the study "need not be large," "be adequate and well-controlled," and "must support labeling for the duration of treatment (6 months)." Titan and Braeburn are working with experts in the field to develop a clinical study design for submission to the FDA within the next few weeks.

Man wollte also in wenigen Wochen einen konkreten Vorschlag machen.
Muss umschichten, in Genvec rein.
