Mal zu PLD. Das Dingens habe ich auch schon durchgekaut.
Abstract
To determine whether pegylated liposomal doxorubicin (PLD) has antitumor activity in pretreated patients with persistent or recurrent endometrial carcinoma and to define the nature and degree of toxicity of PLD. Women with histologically documented recurrent or persistent measurable endometrial carcinoma and with failure of one prior treatment regardless of prior anthracycline therapy were enrolled. PLD was administered intravenously over a 1-hour period at a dose of 50 mg/m(2) every 4 weeks; the dosage was modified in accordance with observed toxicity. Of 46 patients entered, 42 were assessable for response, as three were declared ineligible on central pathology review and one was not assessable for response. Forty had received prior chemotherapy, 11 hormonal therapy, and 29 radiation therapy. Doxorubicin had been given to 32 patients, carboplatin with paclitaxel to six, carboplatin to one, and fluorouracil to one. Four patients had partial responses lasting 1.1, 2.1, 3.3, and 5.4 months; the overall response rate was 9.5% (95% confidence interval, 2.7% to 22.6%). Three of these responses (in liver and in lymph node) occurred in patients who had progressed after doxorubicin with either paclitaxel or cisplatin. The median number of courses was 2.5 (range, one to 14). Toxicity was generally mild: only 25 patients experienced leukopenia, with a median WBC count of 2,900 (range, 800 to 3,900) at nadir. The only grade 4 toxicities were one episode each of esophagitis, hematuria, and vomiting. The median overall survival was 8.2 months. PLD has only limited activity in pretreated advanced, recurrent endometrial cancer, but further trials in anthracycline-naive patients and in previously untreated patients are ongoing. Its toxicity profile should permit its use in combination with myelosuppressive drugs

persistent, wer es noch nicht kennt. Bedeutet die PRs. Die bei der firstline Chemo teilweise angesprochen haben. Halt die zwischen CR und SD. Die sind in der Zoptstudie3 nicht dabei. Nur die CR (recurrent) und die advanced, die dann warscheinlich einer Adjuvanten(begleitende) Chemotherapie nach der OP unterzogen wurden.
Da gehen einige Ansprechraten dahin!Vermutlich auch das OS.

Zopt3 darf unter 2cm.

Das ergibt ein Unterschied von rund 9Mon und nicht nur 3,61! Das Gesamtüberleben ist ja noch gar nicht abzusehen. Und dann noch die Aussage in der Studie: "Doxorubicin may be considered inactive as second-line therapy in this endometrial carcinoma population."
Aber mit "Das Leben ist halt manchmal Schei... " gebe ich dir voll recht!

Second-Line Chemotherapy Experience
Although chemotherapy has shown an important role in high-risk disease, substantial room for improvement exists. Combination regimens are the most active, but in measurable disease populations responses are observed in only ∼50% or patients, and a complete response is infrequently observed. Both the PFS and survival times have been improved, yet the 5-year survival rate for patients with advanced/recurrent measurable disease patients is <10%, and for those with stage III disease it is typically around 50%–60% [19, 20, 22, 25]. After primary therapy with combination regimens, the efficacy of second-line chemotherapy is particularly limited (Table 3). Antimicrotubule agents have shown the most promise. In a population in which 91% had received doxorubicin–platinum chemotherapy, paclitaxel produced a RR of 25%; the epothilone B analog ixabepilone produced an RR of 12% in a population in which 94% had received prior paclitaxel [16, 28]. Interestingly, docetaxel, which had shown activity in paclitaxel-treated breast and ovarian cancer patients, produced an RR of only 8% (80% had received prior paclitaxel) in patients with advanced/recurrent endometrial cancer [29]. . After failure of primary chemotherapy, there is no established active second-line agent in this disease. Understanding the processes by which tumors develop resistance is critical, and defining which patients have the best chance to respond to established or novel therapies is our greatest challenge

Also hier wurde auch nicht abgebrochen und schau dir mal 12 und 24 Monate Überleben an.

Aggressiver geht es doch kaum noch.
Das ist nicht das, was wir in der Zopt Phase2 gesehen haben. Schon gar nicht in der Phase3.
Wenn Dox so schlecht performen würde, hätten sie die Studie zugunsten Zopt abgebrochen!

Sorry,aber solche Einwände überzeugen nicht,in 4 Wochen jedenfalls wirds entschieden,ich jedenfalls werd noch massiv zukaufen

mit Paclitaxel auf Doxorubicin umgestellt wurden.
Das soll kein Vergleich mit unserer Studie sein! Da wollte ich nur das Problem mit den Kreuzresistenten angehen. Doxorubicin spricht nach Verabreichung von Paclitaxel gut an. (Resistenzen)
Die andere Studie die du als Vergleich angibst ist nur in der Überschrift gleich.
Aber auch nur in der Phase 3 bei der wir noch keine Ergebnisse zum Vergleich besitzen!

In unserer Phase 3 sind Adjuvante Chemos zugelassen. Bedeutet nach OP wurde zur Vorsorge eine Chemotherapie durchgeführt. Wie stehen denn die Überlebenschancen bei denen der Tumor bis 1cm Rest oder Kompl. entfernt wurde. Denke wesentlich besser gegenüber Tumore die gestreut haben oder Serös sind.
In der Phase2 waren die Chemos auch Adjuvant!
Denke der Vergleich hinkt!

Results

The median PFS of this cohort following doxorubicin treatment was 2.1 months (95% CI: 0.95-2.7) months. Median OS was 5.8 months (95% CI: 1.0-15.0 months). There is only one patient still alive; her median follow-up time is 49.4 months. Predominant doxorubicin-related grade 2 toxicities included nausea/vomiting (18.8%), fatigue (18.8%), and neutropenia (12.5%). No grade 3 or 4 toxicities occurred.
Conclusions
Among patients with advanced endometrial carcinoma who had received adjuvant paclitaxel/carboplatin, treatment with doxorubicin at time of disease recurrence failed to achieve any objective responses and was associated with a very short (2 months) time to progression. Doxorubicin may be considered inactive as second-line therapy in this endometrial carcinoma population.
übertragen auf unsere Zoptrexstudie wären demnach die Patienten,die mit doxorubin behandelt wurden,rasch verstorben,die verlangsamung der Sterberate wäre der behandlung mit zoptrex zuzurechnen.

Results
Seventeen patients were included in study analyses. The median PFS from completion of paclitaxel/carboplatin was 8.0 months (95% CI: 4.5-13.6 months). At the time of recurrence, all 17 patients were treated with doxorubicin as second-line therapy. No patient achieved objective response of stable disease. The median PFS of this cohort following doxorubicin treatment was 2.1 months (95% CI: 0.95-2.7) months. Median OS was 5.8 months (95% CI: 1.0-15.0 months). There is only one patient still alive; her median follow-up time is 49.4 months. Predominant doxorubicin-related grade 2 toxicities included nausea/vomiting (18.8%), fatigue (18.8%), and neutropenia (12.5%). No grade 3 or 4 toxicities occurred.
Conclusions
Among patients with advanced endometrial carcinoma who had received adjuvant paclitaxel/carboplatin, treatment with doxorubicin at time of disease recurrence failed to achieve any objective responses and was associated with a very short (2 months) time to progression. Doxorubicin may be considered inactive as second-line therapy in this endometrial carcinoma population.
übertragen auf unsere Zoptrexstudie wären demnach die Patienten,die mit doxorubin behandelt wurden,rasch verstorben,die verlangsamung der Sterberate wäre der behandlung mit zoptrex zuzurechnen.

Wie auch immer, die Finanzierung ist erst mal durch. Doddy hat nur im besten Interesse der Fa. gehandelt. Keine Altlasten! Es ist ja auch nicht einfach für ihn, sich auf dem Heimweg immer umdrehen zu müssen!
